Overall Depression and Impairment Scale (ODSIS)
The Overall Depression and Impairment Scale (ODSIS) is a five-item questionnaire that captures the severity and worsening of depressive symptomatology. The questionnaire assesses the most important domains of depression and subsyndromal depressive symptoms, while remaining independent of specific subtypes of mood disorders as defined by DSM-5. The items relate to the frequency of symptoms, their intensity, their interference with work or school and social life. Items ask about experiences in the past week.
Note During administration, it is appropriate to provide respondents with a definition of depression.
Length of filling
Completing the ODSIS takes approximately 3-4 minutes.
Individualized feedback for the respondent
By filling in the scale, the respondent will find out the severity of their depressive symptoms, or their improvement or worsening after repeated filling.

Scoring and interpretation
Responses to items are coded from 0 to 4 and can be summed to obtain a total score ranging from 0 to 20. Higher scores indicate greater severity and functional impairment due to depressive symptoms.
Response scale
The answers to the items in this Czech adaptation correspond to the original version. For each item, there are always four possible answers scored according to severity on a scale from 0 to 4. An example answer could be: “Never. I haven’t felt any depression in the past week." (0) or “Constant depression. I felt depressed all the time.” (4).
